About Us

We are a group of 4 GP Practices working collaboratively to deliver a wide range of services to meet the needs of our population. We have a population size of 31,908 across Ashfield and The Grange Medical Centre, Colton Mill and The Grange Medical Centre, Manston and Scholes Surgeries and Family Doctors within the Leeds 14/15 area.

Working alongside our existing practice staff, we employ a range of other health care professionals such as Clinical Pharmacists, Advanced Clinical Practitioners, Specialist Dementia Nurse, Dietitian, Care Coordinators, Pharmacy Technician, Mental Health Practitioner and Nurse Associates.

Our leadership team includes a Clinical Director, Primary Care Network Manager, GP Frailty and Care Home Lead working alongside practice clinical and managerial leads to coordinate and deliver enhanced services across Cross Gates Primary Care Network

What is a Primary Care Network?

A primary care network is a group of practices who work together to focus on local patient care. They’ve been around since 2019 and 99 per cent of practices across England now work as part of one.

With increased demand and a shortage of clinical staff, practices find they can provide greater, more personalised care for patients when they can work together on certain services or issues.
